Practical classes:
mandatory attendance,
practical completion of the assigned task.
Assessment method:
implementation of the nursing process in accordance with an individual nursing care plan in primary health care at an optimal level.
Criteria:
establishing an individual care plan for the client in the home setting and for their family;
pass – 5 (very good): the student implemented the individual home-care plan and properly documented the nursing process at an optimal level;
pass – 4 (good): the student implemented the individual home-care plan and properly documented the nursing process, with minor, non-material errors;
pass – 3 (satisfactory): the student implemented the individual home-care plan and properly documented the nursing process, with errors that did not compromise the safety of the patient, the family, or the student;
fail – 2 (unsatisfactory): the student was not independent, implemented the individual home-care plan and documented the nursing process but made errors that could have endangered the safety of the patient, the family, or others.

As part of the Kuyavian–Pomeranian Telecare Project (Kujawsko-Pomorska Teleopieka), the student will assess the health status of two patients in the home environment using the application, including: mental status, functional performance, the presence of symptoms such as fatigue, dyspnea, falls, pain, and continence; handgrip strength measured with a dynamometer; and nutritional status.
Practical placement
Professional placement (60 hours) is carried out in primary health care facilities (adult clinic, pediatric clinic, family nurse’s office/practice). Forty hours are completed as part of the Kuyavian–Pomeranian Telecare Project (Kujawsko-Pomorska Teleopieka).
