The course is conducted in the form of practical classes.
During classes in the course ‘Own Offer During classes in the course ’Coordinated Care", students will learn the principles of proper planning, supervision and independent implementation of a comprehensive patient rehabilitation programme. Students will acquire practical skills and competences in the basic clinical skills used in the daily work of a physiotherapist. They will also learn how to improve their clinical procedural skills by applying in a practical way
their knowledge of physiotherapy procedures in various medical conditions. After completing the course, students should know the principles of working in an interdisciplinary team, be able to describe basic clinical procedures in theory and perform them in practice. In addition, they will learn how to develop an attitude of openness and sensitivity to the needs of others and will be able to practise decision-making skills in difficult situations.
During the course, students will be introduced to models of coordinated care in Poland and around the world, developing skills in planning and coordinating the provision of health services, taking into account the criteria of quality and effectiveness.

Term 2024/25:
During the "Coordinated Care" course, students were introduced to a detailed description, supervision, and implementation of a comprehensive client rehabilitation program. Students have the opportunity to apply and utilize physiotherapy functions to the extent possible. They will also learn how to improve procedural skills, key to practical approaches. Information on physiotherapy treatment for various disease states will be provided. Upon completion, students should possess interdisciplinary principles of operation, described and applied in basic application. Furthermore, they will learn how to prepare for emergency situations that may arise in other situations. During the course, students were introduced to coordinated care models in Poland and internationally, developing skills and coordinating the transfer process resulting from limitations in quality and efficiency. Exercises: 1. Medical simulation, medical education in physiotherapy, application to coordinated care issues. 2. Coordinated care in selected disease entities – Neurology. 3. Coordinated Care in Selected Diseases – Cardiology Exercises: The requirements for passing the theoretical and practical sections are: Solving at least 60% of the points in each section. The final grade is the sum of the points from the devices, instruments, and extended alternative sections. |

Recording: developing a coordinated care plan for a specific case: W1 – W2 (0-5 points,
<60%)
Activity during consideration of the discussed topic, its presentation: W1 – W2, U1 – U6 (0-10 points, <60%)
Extended value: K1 – K3 (0-2 points, <60%)
The requirements for passing the practical and hands-on sections are a minimum score of 60% in each section.
The final grade is the sum of the points from the practical, extended, and advanced sections.
<10 - failing.
